Application process
Application process requires that the referral source or individual interested in the service reach out to Monroe County Department of Human Services Preventive Unit at 585-753-5821 and let them know they are interested in the Teen Hope Program at Mt. Hope Family Center. The person on coverage will find out if they qualify for the service and if we have any openings. If both those are a yes then they will walk them through filling out a referral that is then sent over.
