The âItâs About Youâ campaign focuses on improving Infant and maternal health outcomes through a life course perspective, reaching preconception (before initial pregnancy), prenatal/postpartum, and interconception (between pregnancies) clients. It strives to empower women and men to take control of their health, through the dissemination of health education materials, connection to supportive services within their communities, and advocacy. Community Health Workers, home-visiting, risk assessment, client-centered service plans, referrals, preventative screening and health coordination are all components by which this is accomplished.
