Community Health Center of the North Country offers medical clinics which provide primary care, physical therapy, foot care, WIC, substance use disorders, diabetes self management program, pediatric care. Behavioral health is also made available, which involves case management, care coordination, counseling services, OPWDD waiver service consumer direct care. Acts as a safety net for those who are not under-insured.
