HOME AND COMMUNITY BASED SERVICES WAIVER PROGRAM (HCBS) | NORTHERN RIVERS FAMILY OF SERVICES
Assists families with individualized in-home care coordination services for youth ages 5 to 18 years with mental health challenges.
Services include intensive in-home services, crisis response, respite services, family support, information and referral, family peer advocacy, and skill building.
Coordinates individuals plans with the participant, parent-caretaker/ legal guardian, individualized care coordinator, and other supporters identified by the child and family.
