A caregiver navigation program providing supportive crisis counseling, care coordination, comprehensive needs assessment, linkage to community services, education, advocacy with family or service providers, development of a caregiving plan, and follow-up for caregivers. A caregiver is an unpaid individual (whose role can be that of a spouse, partner, family member, friend, neighbor, or other) who is involved in assisting another adult with activities of daily living and/or medical tasks.
