211 Caregiver Project

A caregiver navigation program providing supportive crisis counseling, care coordination, comprehensive needs assessment, linkage to community services, education, advocacy with family or service providers, development of a caregiving plan, and follow-up for caregivers. A caregiver is an unpaid individual (whose role can be that of a spouse, partner, family member, friend, neighbor, or other) who is involved in assisting another adult with activities of daily living and/or medical tasks. How caregiver navigation works: 1) Caregiver contacts 211 2) Intake and assessment 3) Service planning and education 4) Connection with resources 5) Follow-up and assessment 6) Re-engagement 7) Caregiver success Available 24 hours a day, 7 days a week. Over the phone translation available in more than 180 languages.
