Provides Intensive Case Management(ICM) for seriously disturbed children and youths, ages 5-17 years, who are having difficulty functioning in the community. The program is designed to support families in their efforts to help their children, reducing the need for hospitalization and placement by providing weekly home visits, assistance in daily living, advocacy, referral, crisis intervention and coordination of services.
