Description
Provides home health care programs for the chronically-ill who are essentially homebound because of physical limitations. Services include assessment by a community health nurse, individualized treatment plans, nursing care, physical and occupational therapy, speech therapy and audiology testing, respiratory therapy, nutritional planning, social work counseling, home health aides, homemakers, housekeepers, personal care workers, medical supplies and equipment and 24 hour telephone availability. Additional services include housing modifications, home maintenance, moving services, social day care, respite care, emergency alarm response service and congregate meals. Also offers medical transportation, laboratory work, x-ray, drugs, dentistry, ophthalmology, podiatry, medical day care and discharge planning.
