Description

Provides 16 bed unit to care for individuals and families facing the final stages of terminal illness. Services include: medical and nursing management of physical symptoms, counseling services of a medical social worker, bereavement and pastoral care coordinator, Home Health Aide Services, dietary consultation, physical, occupational, speech and respiratory therapy, medical supplies, equipment and laboratory services, prescription drugs for pain and symptom control, short-term in-patient admission for symptom management and respite, and bereavement support groups.

Provides in-home hospice care for residents of Long Island. Services are based upon individual needs, patient and family preferences and the direction of a physician. Home Care services include: medical direction, coordination of services, interdisciplinary care, pain and symptom management, intermittent visits by a registered nurse, social worker, pastoral counselor, home health aide and volunteer, nutritional consultation, physicial, occupational and speech therapy, 24hr/7day emergency assistance by on-call nursing staff, medical equipment and supplies. Inpatient care consists of short term inpatient care authorized by Hospice periodic visits by the hospice nurse, social worker, pastoral counselor and volunteers.
