Description

Comprehensive home and residential care for terminally ill patients and their families, including care in area nursing homes and at area hospitals.Hospicare team of patient's physician, primary nurse/case manager, social worker, home health aides, chaplain, volunteers, Hospicare medical director and bereavement counselor develops individualized plan of care with: pain and symptom control counseling for patient's and family members' emotional and spiritual needs respite care and brief in-patient stays for periods of critical need support and education on what to expect personal care services problem-solving help for legal, financial and family issues companionship bereavement support individual and group services grief education materialsHospicare covers cost of all medications related to the terminal illness and equipment for patient safety and comfort.

Hospicare & Palliative Care Services serves residents of Tompkins and Cortland counties.Palliative care program provides specialized medical care that focuses on relieving symptoms and stress at any stage of a serious illness. The goal of palliative care is to improve quality of life for both the patient and the family. Palliative care is provided by a specially-trained team of doctors, nurses, and other specialists who work together with a patient's other doctors to provide an extra layer of support. makes people comfortable in the last six months of their lives services are provided in homes, in acute and chronic care facilities, and at agency six-bed residence. Following a patient's death, supporting the family with 13 months of bereavement services. bereavement support services to anyone in our service area who is grieving the death of a loved one, whether or not they died on hospice.
