Project ACT - Assisting Care-giving Teens | Buffalo City - Buffalo Public Schools at 820 Northampton Street

An outreach program that provides services to pregnant and/or parenting teens (mothers and fathers, including their caregivers) attending Buffalo Public Schools. Coordinates support services by forming links between families, community agencies, and schools. Also provides resource services and information that supports the physical, social, and emotional development of both the student and the child. Services provided include: - Resources pertaining to pregnancy, child rearing, and home life, including referrals to community organizations that specialize in working with pregnant and parenting teens - Assistance with completing and submitting important paperwork, such as for family court, paternity, child support or day care vouchers, and BPS pre-school applications - Assistance with scheduling and keeping medical appointments - Help with academic needs, including home instruction and tutoring programs - Monitoring of academic and career goal paths and achievement - School and home visitation - Provision of new and gently used baby items

Application process

Students are referred by BPS staff, community organizations, or self-referral. Call for information. Complete a Project ACT Referral & Release form and fax to 716-851-3561. Project ACT Referral & Release Forms can be found on the Health Related Services page under the Departments tab of the District website.
