Fee

Medicaid and most insurances accepted. Uninsured patients may qualify for a state-funded program or a lower fee scale; for a specific list of documents, contact the Health Center. Fees for services are based on household income; bring proof of income to appointment. The following forms of payment are accepted: cash, checks, major credit/debit cards, and money orders. Payment is expected at time of service; payment plans are available for many services.
