Application process

Once an application is received, usually by fax, the applicant is screened over the phone or in person. Applicant will meet/correspond over phone with the intake coordinator to determine eligibility, including funding sources (e.g. SSI, SSD, DSS, self-pay). Please note there is likely to be a wait list for placement.
