DIABETES SELF-MANAGEMENT PROGRAM (DSMP) WORKSHOPS | LOWER HUDSON VALLEY PERINATAL NETWORK
DSMP workshops are free to attend, and are designed for people who have Type 2 Diabetes and receive Medicaid, and meet once a week for 2.5 hours, for six weeks.LHVPN and the WMC Health PPS have partnered with six organizations in the Hudson Valley to offer Diabetes Self-Management Program (DSMP) workshops. These organizations include CLUSTER, Inc. (Yonkers), Yonkers Community Action Program (YCAP, Yonkers), Open Door Family Medical Centers (Westchester County), Konbit Neg Lakay (Spring Valley), Meals on Wheels Programs and Services of Rockland (Nyack), and Rehabilitation Support Services, Inc. (Dutchess, Orange, Sullivan).
