Call 211 to schedule an appointment. Must bring: - Government-issued photo ID - Social Security cards for ALL on the return including dependents - All W2, state unemployment, 1099-NEC, 1099-MISC, 1099-C, 1099-INT, 1099-DIV, 1099-R, and 1099-SA Social Security statements - 1095-A marketplace insurance - HSA statement and expenses - 1098-T, 1098-E, and college invoice documenting all expenses - Letters from IRS including Identity protection PIN - Small business/sole proprietor income and expenses, not including loss or depreciation - Child Care statement with provider address, phone, and employer ID/SSN - Brokerage statements (crypto currency trades are out of scope) - Itemizing documents (property taxes, sales taxes paid, health expenses, charitable contributions) - Any other income information including cash, goods received not as gifts, etc. - Banking information (routing number and account number, not a debit card) For married couples doing drop-off preparation, both spouses must be present to sign at pickup. If this is not possible, the spouses may come to sign at different times, but both signatures are required for the filing to take place. For every dollar donated, $0.85 is returned to the community in programming and 100% of every donation remains in Southern Chautauqua County.
