Eligibility
Oswego County residents ages 60 years or older who: - have functional limitations, unmet needs, and limited non-medical needs - can be maintained safely at home - are not eligible for similar services through other funding sources - meet income guidelines (based on income, may be required to share the cost)

Provides information and referral services to senior citizens on housing, transportation, social security, health insurance, Medicare Part D and C, nutrition, food stamps, legal assistance, and other concerns. Sponsors home-delivered and congregate meals and recreation at senior nutrition sites. Helps with fuel assistance and other programs for older residents. Case management services provide comprehensive assessment of elderly needs/circumstances to obtain in-home services, including a care plan and follow up. Health insurance counseling is provided, including assistance with claims. Legal services, such as advice, counsel, and representation, are available.
