Agency info

SPARC

Based out of Stony Brook University, the SPARC Consortium brings together HIV service providers and consumers as well as other agencies that serve women, children, youth and families in Suffolk. The SPARC consortium works to improve coordination of services, and identify needs and develop collaborative programs to meet them. Outreach offices in Central Islip and Riverhead provide linkage to care coordination services and free rapid HIV testing. Respite and transportation services are available for clients of SPARC Consortium member agencies participating in each program. SPARC's Women's Care Coordinators work to ensure that clients are engaged and retained in care and are linked to other needed services in the community. SPARC's nutritionist offers comprehensive assessments, individual and group education. Primary and specialty HIV care for women, children and youth is provided on-site at Stony Brook/Setauket(Tech Park) and various off-site locations including Islip, Commack, Smithtown and Southampton.
