Description
Program assists seniors 60 years and over to remain in their own homes after retirement. Residents are provided with case management, information and referral, volunteer services, and medical case management and must live in the catchment area which is a census track in the Greenlawn area and include some sections of Greenlawn, E. Northport and Huntington. Call for an appointment.
