Patients must meet all the below criteria:Have a confirmed diagnosis of a chronic disease, a life-threatening disease, or debilitating disease OR be seeking screening services related to symptoms or suspicion of a chronic, life-threatening or debilitating diseaseBe in active treatment, had treatment within the past 6 months, or going into treatment in the next 60 daysBe a U.S. Citizen or Permanent Resident of the U.S.Be receiving treatment at a facility in the United States or one of its US territories

Interpretation services available

Patients will complete an intake over the phone to identify key issues they are seeking assistance with. Following the intake, the patient will receive a call from a case manager, to which their case has been assigned, who will begin to work with them on the reported issues they are facing.
