Eligibility
Individuals ages 18 years and older who: - have a primary diagnosis of serious mental illness - are in the process of relocating from a state-funded program (supervised site, hospital, correctional facility) - are experiencing substantial impairments in function - are homeless - have high Medicaid costs or are under an AOT order - have the capacity to live independently
