HOME AND COMMUNITY-BASED SERVICES WAIVER - PATHWAYS | PATHWAYS INC

A strength-based program that serves youth ages 0-21 years with serious emotional difficulties, developmental disabilities (if in foster care), and medical fragility. Enrollment in the program is voluntary and no fees are charged to the family. Staff meet with the family/youth in their home and other locations chosen by the family.

Application process

Children's HCBS program is accessed through a youth's Care Manager. If the Care Manager and the family believe the youth would benefit from and would be eligible for these services, the Care Manager makes a referral to the program. The program is funded through State and Federal Medicaid; however, children with private health insurance are also eligible through a separate eligibility process.
