Assists individuals and families with common transitions and issues facing seniors or individuals with disabilities or chronic illnesses. Services include assessment, care planning, information and referral, and service coordination. Helps seniors and individuals with disabilities manage household bills, write out checks, balance financial statements, read and write personal correspondence, and complete applications, insurance forms, and other official paperwork.
