Description

Voluntary case management program for pregnant and/or parenting teens under the age of 21 to help them: access appropriate medical, educational, legal and social services to develop independent living skills and become self-sufficient receive information on prenatal care, parenting and child development during regular home visits participate in occasional educational and social group activitiesCoordinates panels at area schools of teen parents who talk about the hardships of being a parent at a young age to try to prevent further teen pregnancies.Individualized support for pregnant and parenting young adults through age 24 including:- Preparing for baby information such as prenatal care, safe sleep, feeding, bonding, diapering, and baby care- Services and support are delivered through regular home visits- Child development information to help promote healthy growth and development- Support in accessing appropriate medical, educational, legal and social services to develop independent living skills and become self-sufficient- Parenting information and support to help young parents feel confident in their parenting skills
